Membership Form (Senior Club Only)

1. For new membership applications you will require a proposer and seconder one of whom must be a Committee member. Members are elected after receipt of details at the next scheduled Committee Meeting

2. Membership subscriptions are due for payment before 31st October (or within 4 weeks of being elected a member in the case of a new member) Members will only be eligible for selection after 31st October if the fee has been paid. 

3. Categories:- Full Playing Member £170 (£200), Under21, Student and Junior Member £80 (£90), Social Member (Entitled to play 4 games) £40 (£50) Late payment fee in brackets if  paid after 31^st October. Special cases may be discussed with your team captain and the membership secretary.

4. Payment can be made by Cheque (payable to Sevenoaks Hockey Club – Subscriptions), bank transfer (Sort Code 60:19:02 Account 13696300 quoting your full name) or standing order. (Full playing members only- initial payment of £40 followed by 5 monthly instalments of £26 payable 1stOctober to 1st February inclusive) 

5. The Junior Fee applies to all juniors playing regularly on Saturdays. Juniors playing on Sundays as well should pay a coaching fee to the Junior Club which will be in addition to the above. For details contact Pia Tucker-Junior Club Membership Secretary piat67@hotmail.com

7. Please note that at present, whilst the club has the necessary minimum insurance requirements for damage to third party property as a result of negligence, there is no provision for personal accident cover.

8. In compliance with the Data Protection Act 1998 all data collected is for the sole use of Sevenoaks Hockey Club to assist with the day to day running of the club. No data will be passed to third parties.
